Your First Visit

Thank you for choosing Avant Allergy & Asthma for your family’s allergy care needs. To assist us with your care, listed below are a few things to prepare you for your first visit.

1 Please arrive at least 15 minutes early to complete the new patient registration.

2 Please sign up for the patient portal prior to your visit, you can complete the new patient forms directly on the patient portal.

3 Please bring a valid photo ID and your insurance card (s), for your primary and any secondary insurance.

4 We require a valid credit card to be left securely on file. If you have any questions please contact us.

5 If you are taking any medications or supplements, please bring a list of these with dosing information, or bring the medications themselves.

6 Please note that if you are interested in having allergy testing performed, in addition to your consultation, do not take any anti-histamines for at least 5 days prior to your appointment.

Examples of antihistamines: (brand/generic)

  • Benadryl/diphenhydramine
  • Claritin/loratadine
  • Zyrtec/cetirizine
  • Allegra/fexofenadine
  • Xyzal/levocetirizine
  • Atarax/hydroxyzine
  • Night-time cough/cold medicines (may contain antihistamines)

Please notify your physician prior to your visit if you are currently taking any blood pressure medications (specifically beta-blockers) or are using any eye drops to treat glaucoma.

7 If you are unsure if you should or can stop a medication, contact our office at info@avantny.com or call us at 646-475-1749.

8 Testing can be done on both arms or on the back. Please consider this when dressing for the appointment.
